Our staff regards continuity of care as essential to the high quality of care that has been our hallmark since 1964. Continuity of care is central in our work because it focuses on:
Taking the time to develop a trusting relationship with each client;
Building the relationship by discovering and engaging each client’s competencies, or strengths, no matter what his or her limitations are;
Close coordination and integration of the different services that a client needs;
Developing mutual support among all the people who are providing services to a given individual or family;
Drawing upon working-relationships throughout the community, e.g., with schools, after-school programs, parent organizations, housing and social service agencies, AA and NA, medical providers, businesses, artists, City employees.
In serving people who face all kinds of adversity, we promote this continuity of care in our office visits, group activities, and outreach to promote safe and effective self-management, social participation, and family life.
You could say that the basic “story” of our work is the many problems people bring us, the diverse teams we offer, and the multiple services we provide. Continuity of care is the “music” through which the basic services come to life for our individual clients.
