Continuum of Services: Health Promotion, Problem Prevention and Treatment To promote healthy communities and serve people who have long-term problems of poverty and multiple risks for mental health and substance abuse problems, Somerville Mental Health has developed a model of closely-coordinated professional services and informal support. This model is based on the perspective that when people face severe, long-term personal and family problems, neither formal professional services, nor informal support alone can provide the assistance that is needed. Rather, it is a combination of professional skills and practical assistance that respects the distress that people are going through, while promoting their abilities to get their lives under a measure of control, that contributes to greater self-respect and positive connections with other people. This perspective has come to guide both our clinical services and prevention programs. Promotion of health and wellness – With our Early Head Start program, we assist parents who are trying to promote the healthy development of their infants and toddlers despite the adversity of poverty. Through our Adult Health Promotion program we assist individuals who are working to promote their health through regular medical check-ups, exercise, good nutrition, and stress management. Prevention of serious problems — With our In-School Consultation, and Homelessness Prevention programs, we provide assistance to individuals, families, children and adolescents who have problems coping with school, or are at risk of becoming homeless. Assessment, treatment, and management of serious problems – Through our extensive Clinical Services we assist families and individuals who are at risk of losing their housing due to behavior problems that they may not initially acknowledge; children and adolescents who have behavioral problems that interfere with their school performance, supportive relationships, and self esteem; individual adults who have disorders of mood, thinking, or impulse control that interfere with their safety, self esteem, and/or competence in tasks of daily living; and individuals who are open to treatment for substance abuse to get their lives under control. |
